"Justice delayed in justice denied" - why are court cases settled so late?

There is a common saying in the legal world, "justice delayed is justice denied". But still so many court cases are held up or go on and on for years and years. So often the people fighting the cases pass away and the case still continues. This is so sad. What is the reason for such delays and how can this be fixed?
